We are looking for a dedicated and compassionate Community Health Worker to join our healthcare team. The Community Health Worker will be responsible for helping community members to navigate and access community services, and adopt healthy behaviors. The successful candidate will work with other healthcare providers and community organizations to increase access to care and improve the quality and cultural competence of service delivery. They will also advocate for individuals and community health needs, and work on a team to plan, implement, and evaluate programs to address those needs. This role requires a deep understanding of the community’s culture and resources, health and social service systems, and health topics related to disease prevention, chronic disease management, and health promotion.
