We are looking for a dedicated and experienced Clinical Case Manager to join our team. In this role, you will be responsible for coordinating and providing care that is safe, timely, effective, efficient, equitable, and client-centered. You will handle case assignments, draft service plans, review case progress and determine case closure. You will also help clients achieve wellness and autonomy. You will facilitate multiple care aspects (case coordination, information sharing, etc.) and work collaboratively with a multi-disciplinary team to provide high quality patient care. You must be able to work in a fast-paced environment, have excellent communication and organizational skills, and a passion for helping others.
